Tested both hellhound quest and larva face quest and they both give a dupe of the item. It's not intended so it'll be classified as a bug. We're working on fixing it right now so it doesn't dupe and we'll be taking away the duped items after it's fixed. dear gms,may i ask something about fixing bug...

can gms tell us its a bug or its not a bug?
and i think karvinen is right. do that quest should not give u a second one.
btw, i think cns left may be laggy and the "corruption" they said.
